As the brain matures, experiences prune excess neural connections while strengthening those that are used more often. Many scientists think that this process contributes to the steady reduction in gray matter volume seen during adolescence (depicted as the yellow to blue transition in the figure). As environmental forces help determine which connections will wither and which will thrive, the brain circuits that emerge become more efficient. However, this is a process that can cut both ways because not all tasks are desirable. The environment is like an artist who creates a sculpture by chipping away excess marble; and just like bad artists can produce bad art, environments with negative factors (like drugs, malnutrition, bullying, or sleep deprivation) can lead to efficient but potentially harmful circuits that conspire against a person's well-being. No matter how long your family member remains living in residence at our treatment program, it is always recommended that he or she follow up with intensive aftercare treatment upon returning home. Studies have shown that individuals who take advantage of continuing therapeutic services like 12-step groups, personal therapy, acupuncture, yoga, and other options have a much greater chance of staying sober longer with no or minimal relapse than do those who do not utilize such programs after traditional treatment is complete.
Traditional alcohol treatment programs rely on evidence-based strategies such as psychotherapy, behavioral modification therapy, peer group counseling, nutritional counseling and 12-step programs. Rehabilitation begins with detox, a cleansing process that allows the patient to withdraw safely and comfortably from alcohol. After detox, the patient participates in a structured series of therapies that are designed to help him or her modify destructive behaviors and create a sober life.
Repeat on each part of your body that’s experiencing the craving. What changes occur in the sensations? Notice how the urge comes and goes. You’ll likely notice that after a few minutes the craving has gone. The purpose of urge surfing is not to make cravings disappear, but to experience them in a new way. However, with practice, you’ll learn how to ride your cravings out until they go away naturally.
Drug addiction is a disease of the body and brain. Also called substance use disorder, drug addiction triggers uncontrollable behaviors and renders a person unable to control their use of medication, alcohol, cigarettes, or drugs—whether they are legal or not. Addictive substances such as nicotine, alcohol, opioid medications, and marijuana are considered drugs just as much as heroin, cocaine, or methamphetamine. Once you are addicted to a substance, you will feel compelled to use it, regardless of the damage that use does to your body, your brain, and your life.
People who misuse alcohol are often addicted to the act of drinking, as much as the alcohol itself. For that reason, you may need to learn skills and coping mechanisms to help you avoid alcohol once you leave a treatment center or return to familiar environments where the urge to drink may be stronger. Your doctor may refer you to a counselor or other treatment program to help you learn those skills and coping strategies.
“There is no conclusive evidence that the drug effects of marijuana are causally linked to the subsequent abuse of other illicit drugs,” states the National Academy of Sciences’ Institute of Medicine. The Institute goes on to assert that underage smoking and alcohol use seem to better fit the profile of gateway drugs. The Institute points out that nicotine and alcohol typically precede marijuana use. This may be true by virtue of the fact that cigarettes and beer are often easier to obtain than marijuana.
Many addicts feel the need to lie about their problems, whether it be to themselves or family members. A group therapy session creates a support mechanism in which patients are encouraged to open up about their issues in a safe setting. Since every member of the group understands that the patient is going through, there is no need for the patient to hide anything or lie.
As you discharge from inpatient treatment, you will receive recommendations for follow-up care and ongoing recovery support to strengthen your sobriety and reduce the risk of relapse. Like diabetes or hypertension, addiction is a chronic disease. Regaining your health means learning to manage your symptoms, first within the structure of an inpatient rehab program and eventually in your home environment where you are in charge of maintaining and strengthening your recovery.

Biological factors that can affect a person's risk of addiction include their genes, stage of development, and even gender or ethnicity. Scientists estimate that genes, including the effects environmental factors have on a person's gene expression, called epigenetics, account for between 40 and 60 percent of a person's risk of addiction.27 Also, teens and people with mental disorders are at greater risk of drug use and addiction than others.28

Nalmefene, an opiate antagonist that is similar in its chemical structure to naltrexone, is one of the most recent drugs being investigated for the treatment of alcoholism. Like naltrexone (sold as ReVia, Depade, or Vivitrol), nalmefene deprives the person struggling with substance use of the pleasurable feelings associated with drinking. But nalmefene is less toxic to the liver than naltrexone. As of 2013, nalmefene was still undergoing clinical trials through the U.S. National Institutes of Health before receiving FDA approval.
